When night watch began, Bridge was as quiet as ever. Radar screens gleamed in the dim light, and indicators on the DP desk flickered regularly. Outside, the sea wasn't entirely calm, but the operation still seemed possible. The wind was picking up. The swell was noticeable. Nearby asset lights seemed steady in the darkness.
• Auto DP was active.
• Thrusters appeared online.
• Generators were running.
• Position Reference Systems was selected.
• The ASOG file was in the bridge.
• Operation was still in progress.
• Everything seemed under control.
But a good DPO knows this: The most dangerous moment in DP is when the system still appears to be "working," but the safety margin is quietly eroding. Because DP incidents often don't start with a single major failure.
Sometimes it starts with a minor loss of line of sight. Sometimes it escalates with the decision to continue when the vessel is nearing its capability limit. Sometimes a thruster is taken out of service for maintenance and no one realizes the vessel is now outside of post-worst-case failure capability. Sometimes water in a fuel system silently hits the power plant. And sometimes ASOG exists… but isn't applied.
That night, Vessel began to speak. But the question was: Could DPO have heard it before the alarm went off?
First Hour: “Class 3 Vessel” But “Class 2 Mindset”
When Watch was taken over, the vessel appeared strong on paper. It was built to DP Equipment Class 3 standards. It had redundancy. The power plant was powerful. The thruster arrangement seemed adequate.
But the operation was being run as a "Class 2 configuration". This is where the first crucial question arose in DPO's mind: "Does the class that Vessel has and the configuration we are actually using at that moment provide the same safety margin?"
- There is minor corrosion somewhere.
- A vent pipe has been running through the ballast tank for years.
- A FO overflow tank arrangement is configured to affect two redundancy groups simultaneously.
- Purifier settings may still be default factory settings.
- The water content alarm may not be active.
And one day, the weakest link in the system reveals itself. First the fuel system is affected. Then the engine combustion cycle breaks down. Then the switchboards fail. Then the thrusters go out. Then the vessel drifts.
- Are the redundancy groups truly independent?
- Could fuel system contamination affect both sides?
- Are the purifier alarm functions active?
- Are the default factory settings compatible with the vessel?
- Has the FMEA detected this hidden common mode failure possibility?
- Is the DP Class 3 vessel actually operating with Class 3 resilience at that moment?
Second Hour: Line of Sight & False Target Reacquisition
A crane boom moved slowly, blocking the line of sight between the Fanbeam and its reflector. The system "saw" another reflector with similar range and bearing, reacquiring the wrong target and moving the vessel 4.5 meters.
Third Hour: Approaching Capability Limits
Operating at 70% power and 77% thruster capacity under adverse weather leaves no safety margin if a thruster fails. If the capability limit is exceeded, the operation must stop.
Fourth Hour: ASOG Live Implementation
When Thruster No. 1 was deactivated for cooling pump maintenance, single fault tolerance was lost. ASOG Yellow status was ignored, leading to a position loss following a subsequent power blackout.
